At The Network Collective, we assist clients by applying to job opportunities on their behalf to save time and increase placement success. Some job applications require sensitive and personal information that was not originally collected during onboarding. This form is to inform you, request consent, and give you full control over how we proceed.

Information That May Be Requested

• Full legal name

• Current and previous addresses

• Date of birth

• Social Security Number (only when required by employer systems)

• Employment history details

• Education history

• Responses to job-specific questions or assessments

How Your Information Will Be Handled

• Your information will NOT be requested or shared via email or text message

• All sensitive information will be collected securely during a scheduled phone call with a Service Advisor

• Your information will only be used for submitting job applications on your behalf

• We will not store or reuse sensitive data beyond what is necessary for your services

• We will never sell, share, or distribute your personal information to unauthorized parties
